Medical History
Each and every patient must complete a Medical History Questionnaire when joining our practice. You will be asked to complete a new form annually to make sure that any changes to your medical details are noted in your dental patient file and that we are kept up to date with anything that may affect how we provide your care.
To save time, you can download this form, complete it at home and bring it along to your appointment. Medical History Questionnaire
Smile Analysis
Our Smile Analysis form helps us to tailor our care to match your individual needs. This is a combination of questions about your previous dental history, any anxieties you may have and it also includes a small section where we ask you to tell us what you would like from us in terms of your ongoing dental care. Payments
Before you commit to a dental treatment plan, we will write to you detailing exactly what we plan to provide for you and this will include full costs for each treatment option offered. We aim to be as clear and transparent as possible with regards to our fees and if anything changes throughout the course of your treatment plan, it will be fully discussed with you to ensure your full understanding and satisfaction before proceeding any further.
We accept cash, cheques, most major credit and debit cards and we can also arrange to accept payment via BACS if requested.
Dental Insurance
You may be a participant in a private dental health care insurance programme which could help to cover some or all of your treatment costs. Please do contact your dental insurance provider to find out if your proposed treatment is covered. We are delighted to help in any way possible and we can provide comprehensive cost breakdowns for you to submit to your insurers for approval meaning that all you have to do is sit back and relax.
